GLENDALE, Ore. – A tragic accident in Glendale has led to the arrest of a 55-year-old Wolf Creek man on charges of second-degree manslaughter after a pickup truck crash claimed the life of his 11-year-old daughter on Monday afternoon. Authorities believe driver impairment was a contributing factor in the incident.

According to the Douglas County Sheriff’s Office, deputies responded to a motor vehicle crash on Oregon Department of Forestry land near the 1200 block of Tunnel Road at approximately 1:30 p.m. on March 10, 2025. The vehicle involved, a green Ford F250, was occupied by the driver, Ian Anthony Fleming, and three juvenile females when it veered off the road and rolled multiple times down an embankment.

The violent crash ejected all three juvenile passengers from the vehicle, causing serious injuries. Fleming, who was able to exit the wreckage on his own, attempted to seek medical care for his critically injured daughter. Using a borrowed vehicle, he transported her to the ambulance station on Tunnel Road, where Glendale Ambulance paramedics later pronounced her deceased.

The other two injured juveniles were transported to Asante Rogue Regional Medical Center in Medford for medical treatment.

Following the crash, Fleming reportedly fled the scene but later returned, where he was interviewed by detectives. Investigators determined that alcohol and drugs played a role in the incident. Fleming was arrested on charges of second-degree manslaughter and booked into the Douglas County Jail.

Authorities continue to investigate the circumstances surrounding the crash.
