Adapt Integrated Health Care Notifies Patients of Vendor-Related Security Incident Involving TriZetto
Adapt Integrated Health Care says a vendor-related security incident at TriZetto may have involved some patient info, including names, SSNs, and insurance details. Adapt systems were not breached. Impacted patients will be notified starting Feb. 2026.
ROSEBURG, Ore. — Adapt Integrated Health Care has issued a public notice informing patients about a data security incident involving TriZetto, a third-party vendor connected to Adapt’s electronic medical record system provider, OCHIN.
According to Adapt, the organization learned of the incident on Dec. 10, 2025, after OCHIN notified the healthcare provider that an unauthorized individual had gained access to one of TriZetto’s systems. TriZetto reportedly took steps to stop the unauthorized activity and secure its systems.
Adapt emphasized that its own internal systems were not breached, and said the incident occurred at a vendor of one of its vendors. The organization also noted that not every patient’s information was affected.
What information may have been involved
Adapt said the data involved may have included certain types of personal and sensitive information, such as:
- Name
- Social Security number
- Date of birth
- Contact information
- Certain health-related and/or insurance information
Adapt stated that, at this time, there is no evidence the information has been misused, but the notice was released so patients can stay informed and take precautions if needed.
Individual letters expected to begin in February 2026
Adapt said that beginning in February 2026, TriZetto plans to send individual notification letters to patients whose information may have been involved in the incident.
Patients may also receive additional updates from TriZetto or its vendor Kroll, which is providing notification services, call center support, and identity theft protection services related to the incident.
Adapt said individual patient notices will include details about pre-paid credit monitoring services and instructions on how to enroll.
What Adapt says it is doing
Adapt said treating patients and protecting patient information remain top priorities. The organization stated it is working with OCHIN to help ensure appropriate security measures are in place and to monitor vendor compliance with security safeguards.
Adapt also said it is reviewing its own processes to reduce the risk of a similar incident occurring in the future.
What patients can do right now
Patients are encouraged to remain alert for signs of suspicious activity, including:
- Unexpected bills
- Insurance statements that do not look familiar
- Messages or calls requesting personal information
Anyone who notices unusual activity should contact their health insurer and/or their bank or financial institution immediately.
